Registering with the surgery

To register with the surgery:

• fill out an online registration form
• visit reception and collect a paper registration form

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Named accountable GP

All patients new and old will be allocated a named accountable GP. If you have a preference as to which GP you would like, we will do our best to accommodate this where ever possible.
